Unicare Health Insurance Company offers FIT (PPO), Saver and High-Deductible HSA compatible plans. Par benefits represent participating providers and are based on discounted rated that UniCare Health Insurance Company has negotiated that save you money. All PPO (FIT) plans have identical benefits except for the deductibles chosen. The Saver plan is a safety net health plan with mainly inpatient coverage. High Deductible HSA compatible plans offer tax advantaged PPO plan options. Affordable Health and dental insurance plans are available for individual, family self employed and small business group insurance. 

Plan Highlights 

FIT Plans: UniCare Health Insurance Company offers PPO plans with different deductible options. These plans, called FIT plans, have a choice of $500-$5000 deductibles. FIT plans emphasize preventative care while allowing members to choose healthcare providers from within the network. $300 free preventative care screening, deductible waived. Child immunizations, 100% deductible waived till age 6. Unlimited Doctor Visits with a $30 co-pay, 80% co-insurance. Members pay 20% coinsurance for participating providers 50% for non-participating. Coverage includes office visits, inpatient and outpatient hospital services, and prescription drugs with a $250 brand name deductible. 

UniCare Saver 2000 Plan has a $2000 deductible and a 75% coinsurance. Like the FIT plans, members pay 50% coinsurance for non-participating medical providers. Services covered include 2 office visits, limited lab and X-rays, inpatient services and outpatient hospital services, and prescription drugs with a separate deductible. 

HSA Plans: UniCare Health Insurance Company offers several high-deductible HSA plans. Members contribute to a special Health Savings Account from which they can pay approved medical bills with pre-tax dollars. This might save members money on their income taxes. The high deductibles in these plans are paired with lower premium rates. Coinsurance offered is 80% or 100%.
